To refer or not to refer Working in casualty taught me a lot of wonderful things. Been here for 1 month and 2 weeks and still loving every minute of it.This is pretty exceptional for me. As I tend to hate the department in my 2nd week either its the workload or the people. Usually the latter The road to our lil abode is always open. Welcome From my previous life as a houseman in various other department than ED, I always held the thoughts that ED always admit unnecessary pt to the always overfilled wards. Those ED guys primary diagnoses were funny (but mind you,sometimes there were a few rare gem which tickles me everytime I chance to remember about them). Their managements were not sound. They whacked IV Morphine to everyone and sundry. But now, being part of the ever busy,calamity prone, disasterphilia Emergency and Trauma Department , it was like a revelation. People who think in retrospective cannot see the actual situation or drama that warrant the managements we did in ED. I've received few calls from my juniors regarding why I didnt do this and that. Why I diagnosed the patient this and that. Why I didnt call this or that. Why do I activate this and that.And this is my usual replies " HELLOoooo (with the emphasis of O sound which I knew is irritating hence the tone), that wouldnt change my management here. This is emergency and trauma, I admit the patient you manage! Unless you want to ask me how I came to the dx bugger off" and tut tut tut.. usually I will slamp the phone down the way Rikishi did to his opponent ,butt to face . And do you know that when patients came,how do they look like? Usually limping,like there will be no tomorrow for them, incoherrent. So the patients u received at ward were alert,comfortable,pink,fair hydration,speak in full sentences. Let me tell you, you have us to thank for. Now that she/he is out of danger its your job to sort out what's wrong with them. Thank you. Stop yakking. Diagnose and treat. And if you happen to be a senior or whatever just because you have worked in the department for gazillion of years,the same year Comet Haley last appeared or since the day Malaysia gained its independence that doesnt mean it gave you the luxury to ridicule left and right. Just give your two cents and guide your juniors. I dont think you are infallible yourself. There must be times when you yourself had made mistakes in the past. Or worst still even after attaining senior citizen card you still make mistakes and then have the gut to laugh at others. I pity you. You will live and die alone. I hope all the young doctors today will never let ourselves be like the old timers. Always emo with crappiness (pardon if no such word exists) stamped on their head. And for those who think they are so great (without ever doing housemanship in ETD) please come and work here so I can see how great you are in managing emergency situation. You maybe great or worshipped in your small department but in general setting you are a nobody. So please lets work for the patient's welfare . Ego will bring you nowhere.
